How to get your COVID-19 vaccine certificate

Enter some information to learn how to get your enhanced vaccine certificate (with an official QR code) if you are an Ontario resident.

You can get your certificate no matter where you were vaccinated (for example, at a pharmacy, mass immunization clinic, hospital, or primary care setting) or if you have an active exemption.

You must select a health card type.

Do you identify as First Nations, Inuit, or Métis? (required)

If you are a non-Indigenous partner or household member of someone in this group, select "Yes."

Did you already contact your public health unit and get a COVID ID? (required)

Your public health unit gives you a COVID ID to use when you need future assistance with the Provincial Vaccine Contact Centre.

If you do not have one or live in a recently built neighbourhood, use the postal code of a shelter, library, or other community space in your area.

Get your enhanced vaccine certificate online through the provincial website.

You will need:

  • a green photo health (OHIP) card (you will need numbers on both sides of the card, expired cards will be accepted)
  • a modern web browser (the website is not compatible with Internet Explorer 11 or earlier)

To print a copy of your certificate, you can:

Call the Provincial Vaccine Contact Centre to get your certificate in the mail: 1‑833‑943‑3900 (available from 8 a.m. to 8 p.m., 7 days a week).

Get your enhanced vaccine certificate by calling the Provincial Vaccine Contact Centre.

You can get your certificate online or in the mail (if you do not have a printer or mobile phone).
