Learn what you're agreeing to when you enter your personal health information and contact information into the COVID-19 self-assessment.

About the self-assessment

The online COVID-19 self-assessment tool allows you to enter certain identifying and personal health information related to your potential COVID-19 symptoms. It is not intended to provide medical advice. Consult a health practitioner or your local public health unit if you have medical questions.

How we use your information

Your information is provided to the Ministry of Health. The ministry maintains the tool and all information that is put into the tool. Ontario Health, a provincial agency, acts on behalf of the ministry to facilitate the management of your information.

The ministry will only use or disclose the information you provide in the tool to investigate, eliminate or reduce the current COVID-19 outbreak risk. It will not be used for any other purposes.

Contacting you

Someone from the ministry and/or the local public health unit may contact you if you choose to provide your contact details in the tool. The purpose of contacting you is to provide information to you regarding your COVID-19 personal response, and to collect additional information from you for the purpose of investigating, eliminating or reducing the current COVID-19 outbreak.

Authorized use

The collection, use and disclosure of your information, with your permission, is authorized or permitted under the Health Protection and Promotion Act and the Personal Health Information Protection Act, 2004.

Your agreement

By inputting your personal information and personal health information into the tool, you are agreeing to the ministry's collection, use, and disclosure of this information for the purpose of investigating, eliminating or reducing the current COVID-19 outbreak.

Entering information on behalf of someone

You must be authorized to enter someone else's information into the tool or when giving information to the ministry and/or the public health unit who contacts you for a follow-up. For example, you are the parent or guardian of a child who is under the age of 16 years and can consent on behalf of the child.

You must be authorized under the Personal Health Information Protection Act, 2004 to provide this consent on behalf of someone else.

Proper use of the self-assessment

Do not enter information into the tool that may infringe the rights of any other person or entity.

You must not:

  • impersonate or make any misrepresentations of any other person or entity
  • impersonate any other person or entity, or make any misrepresentations about another person or entity
  • submit information that you know is false or inaccurate
  • use the tool to gain entry or access to information you are not lawfully permitted to access, view or use, or to gain unauthorized entry to the computer networks or systems of Ontario Health
  • interfere with or disrupt any of the computer networks or systems used in connection with the tool
  • violate any laws or regulations applicable to the protection of personal information or personal health information
  • violate any generally accepted internet practices or interfere with any other person's ability to use the tool or any other services provided by Ontario Health
  • reverse engineer, decompile, modify or attempt to discover the tool's source code or otherwise reduce the tool to a human-perceivable form
  • circumvent any technology used by the tool to protect the tool and associated databases

Terminating your use

The ministry may terminate or refuse to let you use the tool. This can be for any reason and without notice.

You agree that even if the ministry does not insist on or enforce an item in this agreement, they still have the right to insist on and enforce that item.

Security

You are responsible for the security of the device, browser, and network that you are using to access the tool. You are also responsible for the security and privacy of your email account if you provide your email address using the tool and consent to receiving email messages.

The government's security practices are continually evolving to ensure that the integrity and confidentiality of information and systems are maintained. The government uses up-to-date security safeguards to protect its networks and websites from misuse, alteration, copying, disclosure, destruction, monitoring or unauthorized sharing of information and damage. The government's safeguards include the use of security software and encryption protocols and involve physical, technical and procedural controls to protect information behind the government firewall.

Audit

Your access to and use of the tool may be recorded and audited to respond to an inquiry from you, and to investigate any privacy breach incidents.

Disclaimer

The ministry is providing the tool on an "as is", "as available" basis. The ministry does not make any express or implied warranties, representations or promises with respect to tool including, but not limited to warranties as to usefulness, completeness, accuracy, reliability, or lack of virus.

No liability

The ministry is not responsible, and will not be liable to you or anyone else, for your ability or inability to use the tool. This is even if the ministry has been advised of the possibility of damages.

This includes any:

  • losses
  • expenses
  • costs
  • damages
  • liabilities
  • causes of action, actions, claims, demands, lawsuits or other proceedings
